Dears,
I solved the issue by copying 4 missing modules files app_cbmysql.so, app_mysql.so, cdr_mysql.so,res_config_mysql.so from another healthy ISSABEL installation to this folder >> /usr/lib64/asterisk/modules . Then restarted asterisk and it worked ok.
